Demo reel - what to info to put on the insert?

Hiyall -

I am putting together a demo reel of some orchestral music to picture, which is basically a DVD with four clips. Any suggestions for the sleeve/insert as far as what to say and/or what not to say? Obviously, my name, contact info, and the movie titles will be on there, but does anyone have any suggestions as to anything else? Do I put "original film music by xyz" even tho the original score is not included?

Any suggestions will be appreciated! Thank you!

I disagree. Tell them a little about your background and the nature of the music. Even a picture is good unless you are repulsive.:)

Remember your CD will probably be sitting on a desk with a hundred others so anything that individualizes you from the rest of the pile is good.
