Music Notation: A Manual of Modern Practice by Gardner Read is my primary reference. If you can't locate it, I can look up a source, hiding somewhere on this computer.

The Gardner Read notation book is a JEWEL. You will never need anything else unless you go into writing in the modern idiom, then I would recommend "Music Notation in the Twentieth Century" by Kurt Stone.
With these two books, you are set for the rest of your life. Believe me, it's true.
