OT: Quickie Notation Help Needed

My choir director has a 10-measure piece of choral music with German lyrics (by Martin Luther) that he needs set in hymnal format. Our choir is going to sing it in two and a half weeks.

I haven't used notation software in some time. Could somebody proficient in Sibelius or Finale please set this for me, and send me the document you create?

My director gave us a printout that he set in some piece of software or other, but it looks quite poor, with wrong note durations, no slurs or fermatas, etc.
