Hi, I have a new (and unused) Whirlwind HotBox Active DI box.

It's got two 1/4" Phono Jack inputs (in & thru) and an XLR output. It can be powered by either +48VDC Phantom Power via XLR, or by 9 volt batteries.

It's also got a battery on/off switch with optional padding (for long cable runs of strong signal) and a ground lift switch (to reduce signal noise inducted from secondary ground connections)
